For those in the community, transfers are a part of almost every shift. There are times where it is very obvious from your first moments in the room: this patient has got to go. But clicking that disposition does not mean 'out of sight and out of mind'. In this brief talk, we will discuss some medication considerations to make the transfer process smoother.
Learning Objectives:
After this session, the learner will describe medication options to decrease nursing burden (antibiotics, anticoagulants etc).
After this session, the learner will list the most critical home medications to order in the patient pending transfer ex. anti-seizure, antihypertensives, rate control medications, antipsychotics etc.
